Diagnostic Assessments

Gain clarity and direction through a compassionate, professional mental health evaluation. Our diagnostic assessments provide the foundation for personalized care tailored to your unique needs and goals.

Diagnostic Assessments at Agape

A diagnostic assessment is a comprehensive clinical evaluation used to determine an individual’s mental health status, identify possible mental health or substance use disorders, and guide treatment planning. At Agape Health & Wellness, this assessment is conducted by a licensed mental health professional who gathers information about your current symptoms, history, strengths, and challenges.

The process typically includes a clinical interview, standardized screening tools, and a review of relevant medical and psychological history. The goal is to arrive at an accurate diagnosis and develop individualized treatment recommendations that support your recovery and well-being.

Diagnostic assessments are often the first step in accessing therapy, psychiatric care, or medication management and can be used for initial evaluations, treatment updates, or referrals to additional services.

What to Expect During a Diagnostic Assessment

When you schedule a diagnostic assessment at Agape, you’ll meet with a qualified mental health provider for a 60–90-minute appointment, either in-person or via our secure telehealth system. During the session, the clinician will ask questions about:

  • Current mental health concerns or symptoms

  • Personal and family mental health history

  • Past treatment experiences (if any)

  • Substance use history

  • Medical history and current medications

  • Strengths, stressors, and support systems

We may also use standardized screening tools to gather additional information. After the assessment, your provider will discuss their findings and offer personalized recommendations for care, whether that’s therapy, psychiatric services, or community resources.
